Tragic but confusing....

This story is obviously horrible, but i just can't figure out in my head how this could possibly happen! I mean of all the ways to go, not in icy conditions after you have cleared your car, but actually while doing it?! I know the report describes how they think it happened, but its just unbelieveable!

Most houses on my road have steeply sloped drives, some are mad, could be a handbrake failure (as I've had) or indeed ice, though I'd have thought the car would have had to have moved so as to be on the ice, so a combination of both?
I've parked my car in icy conditions, got out and walked up the drive to hear/see my car start sliding back (wheels locked-handbrake on+1st gear) down the slope
