The 'pitman arm shaft' has a lever arm on the end, as can be seen in this photo (of a vintage Bugatti, in this case):
The lever arm from the steering box is then linked by a rod to a steering arm on the front upright/stub axle, which can be better seen on this photo of the same car (it's the curved 'horn'-like piece of metal above and to the front of the axle tube):
The two front wheels would then be linked using steering arms and a track rod (think similar configuration to a rack-and-pinion steering system, but with a simple rod from one side to the other replacing the rack and track rods); the track rod can be seen in the above picture - it's the slim rod running parallel to the axle tube, behind it.
