Help me identify this 70's Omega?!

Hi. I recently inherited this watch from my uncle but can't find anything about it, can anybody help?!
It appears to have been purchased in Poland (he worked there at the time) as there is a receipt in Polish, dated the 04.12.75 and a referance number of 005586. There is a seperate tag showing the price of $132 and is all encased in a red Omega presentation box.
Apologies for the poor quality i-phone photo, hopefully it shows enough detail. Printed on the watch face top to bottom are: Omega logo, OMEGA, Automatic, Geneve, Swiss Made.
Unfortunately it doesn't keep going for very long, so guess it is due a clean and service. Can anyone also tell me of somewhere in Birmingham that could do this?
