I think there are several dimensions to that.

Despite his medical reports, objective and reported evidence is that he's an elderly man within 10 years of his father's life expectancy with obesity, poor diet and virtually no exercise. So he's at a fair risk of getting/dying from cardiovascular disease or cancer within that time frame and perhaps in the next 4 years.

His Alzheimers will progress over the next four years. Paradoxically Alzheimers in old age tends to progress more slowly than early onset disease, (IMO it's probably a different disease mechanism) so disease progression may not be any more obvious than the changes mapped over the last 10 or 5 years. The question is whether it the consequences will be contained (like Bidens limitations were) or whether his lifelong experience of getting his own way will make this impossible. Then I suspect the team will continue to sanewash his public comments and behaviour whilst hanging on to the coat tails and pursuing their political agenda.

He has obvious cluster B personality disorder - psychopathic or narcissistic PD (grandiose sense of self-importance, superficial charm, need for admiration, lack of empathy for others, poor impulse control), take your pick. This accounts for the puzzling third party response to him - either you regard him as repulsive and barely human or charmed and seduced. The danger for all of us is that he is heading for a spectacular narcissistic collapse. I hope his body or brain gives up before this happens because he has no guard rails around him and could do a lot of damage.
