I don't generally watch Droo, but I watched this one, as did my wife, who is much keener than I. But she agrees with the below. Actually, she said it before I did.
This ep (the one without the Dr) was claptrap.

Complete bunkum and RTD is relying on the fans not being objective.

As for unanswered questions, yes and no. I think the writer was inviting the viewer to use their own imagination a bit, so maybe there's no right or wrong answer to some of them.
I also think that some of those answers may become clearer in a later episode.

I’m fine with loose ends. Plot holes or sidestepping any semblance of plot are another thing entirely. - what happened to the Dr
- how did Ruby become the old woman
- what did the old woman say to people
- why did a two bob Adam Driver want to be PM
- how come UNIT weren’t perturbed by the Dr’s disappearance / able to tell Ruby what happened

Pretty much sums up my thoughts.I thought it started well, and was suitably unsettling. It would have scared 10 year old me I think.
Maybe it will all become clear in future episodes, but if you're going to do episodic story telling within an arc, it needs to be consistently done or it just becomes confusing.
