I occasionally have the same thoughts about being out running, haven't yet done anything about it. Most running and cycling kit has at least a coin pocket so a small piece of laminated card with emergency contact details on it might be a good idea. I'd make my own rather than paying anyone for it.

Had one of these for a year now. Good quality and prices seem reasonable for a little peace of mind that if I'm not able to communicate for whatever reason then folks can get the right details to get me looked after.I have used these which seem to be lasting well considering the amount of time I spend wearing them.
