I did write a long reply (before my laptop crapped out) but suffice to say a "1" can be achieved from not having paperwork in order. Vac-pacs are not an excuse.

Or I could fill out a pile of paperwork and claim I was visited on "a bad day in between deep cleans" and get a "5".
No defence to the OP's post but the HSE approach to food safety is as much about the paperwork as the practicalities.
I can say this having been in the industry for 20 years and having only seen an inspector 3 times despite working with some of the most vulnerable groups possible.
Think clinical standards with feeds and meals. Not inspected in 5 years.

I did some PT work in a 5* last summer, was shocked at the level of filth and general poor standards, yet other areas they seemed obsessive about (presumably box tick items). This was a supposedly really good pub/restaurant that I knew previously as a customer. No way would I eat there or recommend it now, it's fair put me off and made me cautious about eating out now in general.
There are odd occasions when places get low rating for unfortunate reasons though so perhaps could be redeemed? The was a BrewDog bar recently, can't remember the location, but they got a low score because of a pest issue in the area (rats) and the fabric of the building was deemed unacceptable to stop them getting in, presumably because it was old.
They've obviously now gone to great lengths to rectify this but it's an unfortunate blot on the records. Probably wouldn't stop me going there though.

Looks very similar to the Fat Duck There is a local curry place, which was our default for (10+) years. Always nice food, and never had any problems. However, when I discovered they had a 1* rating, I admit I stopped using them - my friends didn't, but the group curries haven't really happened for a few years anyway.
It's changed hands in the last 6 months, so will be interesting to see if the rating increases.
