Ok so I'm a northerner and I go on a few rules.
If I have to get up from my table to order, get anything or pay I don't tip.
Any service charge is politely asked to be removed and if device is good will be given to the serving staff in cash.
Up here it's not so much of an issue but if I were in a more expensive area I'd be more inclined to top for average service. Up here the minimum wage isn't slave labour and most serving staff are students who dont have many real outgoings.

Northeners, especially Yorkshiremen & scotsmen, are known for being "careful" with money. "Ok, so I'm blue eyed with dark hair and here are my thoughts on tipping..."
Nobody is actually genetically predisposed to being careful with money based on the location they were born!
