The wars in the Riverlands mostly happened offscreen. The Greatjon and that dying etc, the leeching of forces. Lannisters lost a lot at Blackwater too, before the Tyrells showed up.
The Riverlands was weak, hence the Blackfish being able to retake Riverrun. If the Lannisters have more men, they must be back at Casterly Rock with Kevan. I think they're down to the same levels as Riverun/North.
Qyburn, he needs no revenge on the Mountain, he's had that a 1,000fold! Probably subtly chortled his tits off when he got the chance to keep him alive in a torturous state.

Remember the drunk talking about doing to Cersei what he does to his wife, the Mountain then crushed his head against a wall, EP1 I think. Later in the episode when Qyburn is talking to his "little birds" he asks one how his mother is, the boy say she's well now but that's because his dad hasn't been seen for days. He looks after his little birds well, did their former master Varys?

In GoT, it's a glamour when people try and look like others. Using the faces of dead people is a easy toolkit to do it for anyone with he knowhow, to do it with pure illusion requires much more effort/energy and know how I reckon. People we have seen do this are Mel and Jaquen. Level 100 users!

In the books Jaime has a nice arc, I can't quite recall how it is with he and Cersei in the latest one, I think the show is roughly the same, we got a great insight into Jaime's life back in S3, when Brienne was taking him back, the scene in the bathhouse, is one of the best, 'my name is Jaime.'
