The issue I'm seeing is poor governance and a factor is very weak RACI (RACI stands for Responsible, Accountable, Consulted, and Informed. Applies to complex organizations as well as projects. People need to know where they fit in the model. This group seem to hopelessly confused as to what their accountabilities were at any given point, and following the best examples of the civil service it's all been fudged and "looking back now" and "lessons should have been learned".
Weak leadership, weak management, weak governance, terrible advice, probably poor/no effective whistleblowing, too much self interest.

One might suggest that was a deliberate tactic with all the people recruited, promoted and engaged. Bully boy investigators with low morals, useful idiots as managers and lawyers all bonused on the "desired outcome" (namely keep the train rolling) and greedy firms suckling on those juicy fees.The bigger and more bureaucratic the better - hide in plain sight to to speak. I doubt anything will land on PV - it will be "i wasn't told, it's someone else's job" and of course "i dont recall"

I wouldn't trust a single thing that comes out of his mouth...............insincere and smarmy to my mind.Maybe he was looking up at something briefly but council did seem to pause as he looked back up from his notes to emphasise the question, and caught the slight eye roll! Sir Wynn has asked him to clarify a couple of points when weasel features has tried to use a tricksy little ambiguous answer! I don't think he's enjoying his day anymore, he seems to be getting a bit 'tetchy' Long pauses at times while he thinks very carefully about his answer.
