I've been an American Express card member since 1998 and have had a Platinum card, the "highest" offered in Belgium & Luxembourg, for about 10 years.

I used to redeem the membership rewards points for the annual renewal fee, until they upped that to 100,000points, then I switched to redeeming the reward points for Swissrent vouchers. Those covered all the ski stuff for my 2 children and top quality skis for my wife when on holiday in Switzerland. Now they've stopped that, I'll not be renewing in 2011 for €520/year - especially when I saw how much they charged me in FX commission (only shown online, not paper statement) during a 3 week trip to US & Canada in July/August!!

Dexia now offer a much better card with EuropAssistance for both cars, travel/baggage insurance etc. built in for €200/year. (I pay €150/year for a family EuropAssistance travel insurance now, so with cars incl. for €50 extra = bargain for me) Dexia's FX rates are more favourable too.

Its very handy; I use the concierge and travel stuff all the time, and because I travel globally I usually put about $250K a year on it. Airfares and hotels add up fast if you are doing 100,000 air miles annually. As to whether its worth it, hard to tell. I had a platinum for years and i was OK.
