How to fit handheld shower bracket

I'm in the middle of doing up my house and I can't work out for the life of me how I fit the hand held shower holder.
This is how the pipe connection and the bracket look
My understanding is you drill a hole in the shower wall and then insert it and tighten the nut. Unfortunately the thread does not seem big enough as it looks like this once the pipe is screwed on.
Does any one have any ideas?
