Can be frustrating, Maple went 5 times in the house one lunchtime. Each time my wife was cleaning one up she did another! Both now in the doghouse - we got to 10pm with a zero pee in the house tonight. Until......
Consistency, massive praise when they get it right, train a word when they go, (we use "toilet" for wee and "go poo". Handy when you're going into a shop or pub and want then to empty on command.
We kept an eye on her pretty much 100% in the first few weeks and, (just like a toddler) tried to know when she'd need and make sure she was in the garden ready. Also removing the water bowl at 6pm helped getting through the night in the early weeks.
I believe they go where they get used to going, and that each wrong one counts 10 against you. Just like kids, they will get it. Until then, it's Groundhog Day, but you now know to get them in the garden at 5 to 10 tonight.

How could anyone have given her up?!I don't think she spent a lot of time out of the house before. I'm certain she was well loved and showed absolutely no signs of ill treatment, but she was extremely nervous anywhere near people (or even walking by doors, which people come out of!) and would bury herself under the sofa and shake if we had visitors.
She's still timid, but she's a very very different little dog now.
