Relax BERGS. The ear protects itself to an extent with loud noises and you'd need to encounter constant loud noise way beyond the duration of a birth for proper audiological damage. Regular, loud i-pod use is actually more damaging.
You could just tell her to shut her fking cake hole anyway. That usually works.

In my daughters case about 15 seconds. At the conception.

Eat when you can because with the high room temprture and the lack of food can also lead to passing out, that and the wetting of the babies head as well.
I kept my face about half an inch from the OH's all the way through so that I didn't see what they were doing. I'm not squeamish, I just didn't want that image coming back to me later.

The lad in the recovery room next to us was a big, hard-looking fella. Passed out 3 times during the birth rofl They propped him up in the corner and left him to it eventually smile
