Just did a bit of research, and it didn’t take long to find the answer. Apparently, this is a basic mod you can expect to find on stunt bikes – an additional rear brake lever mounted to the left h/bar.

They have these rear brake levers on the IOM TT bikes too so the riders can keep their feet firmly planted
Didn't know that. Presumably further spaced out than the stunt bikes.

Not sure when stunt riders started using it, but in the nineties Mick Doohan was the first GP racer to use a left hand thumb lever after his horrendous right leg injury left him unable to use his right foot. He even went so far as to have his ankle fused to help him ride.

Really?
Must be a new thing as I have never seen that, and I used to live there right next to the pit lane.

Some teams may use them but I am not aware of any seniors that do it.
Happy to be educated.

(Personally you won't get any "feel" using a finger rear lever)
I only became aware of it in this year’s coverage but this article is from 2019 so has been going on a while. Hickman has to class as a senior right?

Is it possible to continue this utterly fascinating bike brake pedal debate over in the biker banter thread? Thank you. Nobody cares.
