Has anybody else had a problem with switchgear coming loose on the handlebar, causing loss of control of the bike?

The switchgear on my bike came loose and the throttle cable got caught behind the brake lever. Hence, no brakes. Hence, big crash.

Currently investigating whether manufacturer can be held responsible. It was a fairly new bike (5 months old). Insurers are being a pig about releasing the engineers report though. I might have to get another one done, which will be paid for by the legal expenses insurance anyway! Manufacturer could end up paying out for all uninsured losses, including time off sick (I'm self-employed) and injuries.

That would be naming and shaming, and I can't do that until Yamaha admit liability.
