"Failed Payment" - but nobody knows?

Sounds to me that the most likely answer is confusion with the fraud department.
As far as I am aware if you report a lost card the fraud department immediately suspends/holds/stops any unusual payments (and subsequently gets your authorisation for them or returns them to you)
My guess is that this department has refunded you the amount and the amount is sitting there as 'stolen' by the car company to be investigated at a future date.

+1 for the card payment being authorised but not settled.
When card transactions are made usually an initial authorisation hold is placed (some banks will call this a pending transaction) which affects your available balance but not your account balance. In most cases the transaction is fully processed (claimed) within a matter of days, where the authorisation hold is removed and funds will actually leave your account and will show as a line on your statement.
Where card payments aren't "claimed" in a timely manner the authorisation hold will automatically expire. But the transaction can still be "claimed" at any time within 6 months.
The funds will likely debit soon. If you don't leave the money in your account you may soon find yourself overdrawn and incurring bank charges.

I agree with thoughts above. You've done your bit, you've tried to rectify. So put money aside, invest it and if they ever ask for it they've got it.
I had a client pay me a random £300 a few years ago. Despite asking them what it was for they denied paying it. They didn't want it back because they hadn't paid it so wouldn't be able to account for it, said I must be mistaken. So I kept it , if they ever ask for it, unlikely now it's several years later I would happily pay it back.

Well it has been a month now since I made the payment and the money initially left my account.
Since my last post I have contacted both car company and bank again. I have had a statement of accounts which I requested from car company. It shows the amount paid off, with a rebate of 81 pounds (whatever that is, I presume less the interest for clearing the balance early).
I am starting to believe now that I may end up with a car that should have cost 8 grand in total, for 3.

Keep quiet about it, keep the money aside for a year or so. After that, I'd call that quids in.
if you keep banging on about it and raising it as questions to the car company it will eventually cost you the £4500. if you have money to burn, keep going!
Worst case, 22 months from the date of the transaction (because they have to file accounts within 10 months of their year end). This should be detected at audit time. If it's not (or they decide to write off discrepancies that small) then I think you're done, especially with repeated evidence that they believe you've settled. But i'd wait 'til the relevant accounts have been filed before I spent the money.

I'm not aware that's ever happened to me, but it used to be a regular thing with cheques - I got my car insurance free one year, a hefty amount back then.Colleague paid for a holiday with a credit card and it didn't get charged to his account until 12 months later. He complained about it, but was given short shrift.
