I've found the TOPEAK Survival Gearbox to be very inclusive and have carried one for years, it has a patch kit, tire irons and chain breaker and can be clipped to a seat post or tossed in a small bag. With it and a pump you can handle anything that can be repaired in the field.

As a bare minimum: mini pump & glueless patches.

If you have a tiny bit more space (like in a Camelbak or secure pocket) then add a spare tube and a multi-tool.

I carry 2 tubes, repair kit, multitool with chain splitter, tyre leavers, bent piece of coat hanger to hold broken chain together when repairing, 3/4 links of old chain and some change wrapped in clingfilm

Never been stranded yet even when rear derailer was ripped off and chain snapped in the middle of the Lakes miles from anywhere.

Add a mobile phone in a sealed bag to that list for my peace of mind. Preferably a phone that won't be missed if it gets wrecked.

I take:
-Tyre lever + puncture repair kit
-Chain splitter
-Allen keys
-Small adjustable spanner
-Pump
-Mobile
-First aid kit

That's when I'm MTBing. For road, I take my mobile and that's it.
