Reporting a old age driver?

My late Dad spent a career driving. He was a test driver for a tyre company driving all over the USA and up and down UK motorways daily. He was an Advanced Driver and had done courses in everything through his job. He'd had one accident in his life and it was someone rear ending him at lights.
He was diagnosed with cancer in his late 60's and obviously became a little frailer. One day he was driving Mum to Tesco when he clipped the wing mirror of a parked car. To say he was mortified was an understatement. He couldn't comprehend that he'd done it. A meeting was called and he asked if I minded taking over the driving from here on (my Mum never passed a test). He absolutely refused to drive again and said he didn't feel safe.
True to his word he never got behind the wheel again.

He was a great bloke my Dad and a great role model. We didn't always see eye to eye, especially through my teens but he always knew what was best for me even if I didn't.It sounds like he was a strong and principled man to voluntarily do that
Oh, and he didn't let me buy that 2CV at 17 that was sprayed up to look like a Ladybird
