Slightly off topic, but I have seen a piston in a diesel bus engine break a chunk off the cylinder head and both chunk and piston end up inside the passenger area having burst up through the floor panels.
The bus was a learner vehicle and was trundling down the dual carriageway when it happened.

Very interesting writeup, but the main issue in this thread is, did the pistons stay inside the engine or did they fly dangerously around the car endangering track supposts. Crazy engine above 5k ??
