Mini One R56 Overheating

Hi all,
An issue with my sisters Mini is that it has started over heating while driving. I've exhausted my knowledge and know the following..
Car went in for an MOT and the tester said he struggled to get it up to temperature (not sure if this is just a coincidence or they have done something?)
She's complained the air con isn't cool.
Thermostat replaced a few months ago.
Low speed fan doesn't kick in when air con is running or when temperature gets to 108C.
What's threw me of is the car only over heats when driving. I'd have thought it would have cooled it down having more air flow?
It seems everything points to the fan but I'm not 100% because I checked power to the fan with the air con running and wasn't getting 12v. I've also swapped a relay out but is there 2?
