I did, although I'm not 100% sure I needed to, I did as a precaution It is asking you to get a witness from a list of acceptable people (e.g. no doubt solicitor, doctor etc.) to complete section 5 confirming that you are who you are as per your driving licence.

That looks like a DBS application. I used to fill in similar at work. Designated person could be MD or official HR person confirming they've seen documents, space for company stamp adjacent if got one. In my case, I used to get it all filled in and put it in front of MD and say "sign this"
