Yet another vote for the Johhny Cash version of hurt, plus these:

Dan Reed Network - Long Way To Go

Queen - These Are The Days Of Our Lives (video was recorded when Freddie was close to death)
http://youtube.com/watch?v=YOJPvxgkvn8

Three Doors Down - Here Without You
http://youtube.com/watch?v=TqRAL19MrDU

Daughtry - Home
http://youtube.com/watch?v=3AliYh7oWIU

Within Temptation - Memories (a particularly poignant video)
http://www.youtube.com/watch?v=3tAonkT3iBk

Damien Rice - 9 Crimes (even if I don't understand what the hell is going on in the video)

something a little more modern, you sentimental old farts! Not a classic tear jerker, perhaps but think this touches the spot more than most because of things which were happening in my life when this came out.

I cried like a little girl the first time I saw that.
Indeed, the one and only song I can think of atm that has that effect.

World in Union (IRB "anthem" ). Again, very subjective as it was from when was having the best 3 weeks of my life in Australia in November 2003. Sat behind those posts in the final only scratches the surface of what that trip means to me.
