It's not a plothole. The knife can be directed by the person who is holding it, and the waitress needs to time it right.

But the person holding it was not in on the scam. He was the one being scammed.

It's obviously well oiled scam. The target needs to get the knife from person A, person B is always going to be in the zone to act. Person A can direct the motion enough to allow person B to move with synchronacity.

Someone far cleverer than me is going to have to explain the very last clue, about the Prof's middle name being Hector and the circled letters on the crossword...

Edit: Reading the Guardian article on tonights episode (the grid is also todays cryptic crossword) gives the answer to the above question.

I enjoyed that, though I always thought that "deduction" in the sense of solving a riddle or clue came from "to deduce", but am sure that he said "to deduct". Erased it now, so I can't check. Like a nice crossword, though, so that was quite good.
