In the opening shots of A Hard Day's Night when the boys are being chased by adoring fans
George is running full pelt when he falls hard on the pavement, I'm sure it wasn't in the script
but it was well worth keeping in.

Any other film/tv examples of unscripted events that added to a scene.

Star Trek. When Scotty bumps his head after saying 'I know this ship like the back of my hand'.

A stormtrooper bumps their head in one of the star wars movies too.

In the opening scenes of Saving Private Ryan, the troops who vomited on the landing craft, it was genuine seasickness and kept in because it added realism.

Tom Cruise breaks his ankle in one of the mission impossible films and then gets up and tries running on it which is pretty impressive.

Leonardo DiCaprio smashed that glass in his hand in Django unchained and finished the scene.

There's a scene in The Long Good Friday when Charlie and Harry are driving away from the airport - Charlie goes to speak but Harry cuts over him and Charlie stops. It's definitely a gaff as it adds nothing to the scene and jars with the flow a bit. Don't know why it was left in.

“You talkin’ to me?”
“Game over man”
“Here’s Johnny”

I bet you all know all of them.

I seem to recall a lot of Robin Williams' radio show in Good Morning Vietnam was ad libbed.

You could see the way Kerry Washington looks off camera, almost asking 'what's getting smeared across my face?!' when he wipes his bloody hands on her.

Joe Pesci’s

“Funny how? Like a clown?”

Speech in Goodfellas was partially improvised, none of the other actors knew where he was going with it.

Smiler.

I only noticed the shooting star, in the ITV screening a few weeks ago.

The scene in that other of Shaw's nautical adventures, The Deep, where Jacqueline Bisset's stick is grabbed by the Moray eel, is rumoured to be unscripted.

In the film “Leon” Gary Oldman plays the baddie and in an encounter with a character who was suspected of stealing drugs starts sniffing him, very close up... the other actor looks bemused and shocked - the move was unscripted by Oldman who thought it would add menace.

Harrison Ford shooting the saracen in Raiders of the Lost Ark.

The entire cast and crew were suffering from the sts and when the saracen had finished giving it large with his sword there was meant to be a big sword fight vs bullwhip scene but there was no way Harrison Ford could perform the fight scene with liquid guts so Ford and Spielberg decided to 'Just shoot the sucker'

The lineup in Usual suspects was filmed after a boozy lunch, the sniggering after Baldwins line is genuine.

Dumb and Dumber
In the van, when Lloyd asks Harry if he knows what the most annoying noise in the world is.
If you watch it, you'll see Jeff Daniels is surprised when Jim Carrey asks him as he wasn'tcexpecting the line.

Zoolander, when Derek asks David Duchovny's character 'but why male models' for the second time..it's because he forgot his lines. Bloody funny scene because if it !
