During the storm sequence in Ryan's Daughter veteran actor Leo McKern was only meant to in the water for a few
close-ups and a stand in would do his more risky stuff.

Nature thought differently and a huge rogue wave caught everyone unawares and while the cameras were
running Leo was washed out to sea , luckily they had thought to tie a rope round his waist just in case.
David Lean thought it was great footage and kept it in the final cut.

Contrary to popular belief, the actors in Alien did know the chestbusting scene was going to happen. They didn't expect to get sprayed with blood though, so those reactions are genuine. Or when Hans Gruber falls at the end of Die Hard, the Director promised he would be let go on the count of 3, but instead did it at 2 to capture a more realistic reaction.

One of the long takes in Children of Men has a bit of (fake, obvs) blood splatter across the camera lens right at the beginning. Knowing that there's so much left of the take, the director supposedly had drawn the breath to shout "CUT!" when he changed his mind and let it run. The blood stays on the lens for the entire scene and helps to make it one of the best ever long takes.

The shooting star was, i believe, a regular Spielberg ‘thing’

your pick of the lines..
Another unscripted moment in a Spielberg film. Drew Barrymore reaction to the alien in E.T. “Is it a pig? It sure eats like one.”
Or that could be a story I misremembered.
