The small govt thing often shifts the burden to the states.

e.g. Vehicle Inspections (MOTs) which vary by state (?) and creates a complex web of legality once you start moving cars between states.

Culturally the US seems to equate a federal standard with big govt, which isn't always the case. You can have a small govt setting good standards for states to execute against.

Seat belt laws are another example. Mandated that they are fitted. Some states don't mandate any usage.

It's quite variable between states; in California most things cause cancer, up to and including really big wasps and rising intonation, whereas Texas expects poor people to live downwind of chemical works and oil refineries and keep quiet about the funny looking kids and the whole dying before getting to pension age thing, not that poor people in the US have pensions.

And the top accomplishment ........ "ending the COVID-19 pandemic"

Do we have cancer-causing levels of contaminated drinking water in the UK? I'd be surprised if any country in Europe does

In terms of deaths due to unsafe water or unsafe sanitation, surprisingly, Germany, France and the UK are the worst in Europe. In 2016, 480 people in Germany due to bad water/sanitation. France and the UK were 172 and 130 deaths respectively. By comparison the Ukraine had 119 deaths, Belarus had 5 deaths. This is despite both countries sharing a Chernobyl contaminated zone.
