Had an Amex card since 1980 and been in BA Avios and prior Air Miles since they began. The combination of Avios and companion voucher when there's two of us travelling in, say, club, all for the tax of maybe £100-£200 far outweighs their forex rate, which I find OK anyway,

Never had any real issues regarding non acceptance of the card. There are one or two retailers in the UK who refuse it but I seem to always buy using this card anyway.

Paid 4 grand for BA flights with my BA Amex credit card and they charged me 1%.
The fee is charged only to UK purchasers.
Funny how I regularly move to Ukraine for a few minutes whenever I am buying BA flights.
How do you do that?
Change country to Ukraine on the payment page (or to any non-UK country, which is the only one they charge a credit card fee..).

But then doesn't the website convert the price to local currency, meaning that when you pay with your GBP Amex you're hit with the ~3% FX commission + exchange rate?

No.
