What do we all use then? My travel is mainly restricted to Europe and ANZ ...

I was pondering about taking a simple 4 gang extension cable and a basic adaptor, that way, I don't need too many adaptors, but thinking of also taking one that can do two USB devices (iPad and iPhone preferably).

or is there a backup battery type device that allows you to charge two devices, whilst charging the battery at the same time?

That's what I do, although I'm mainly going the US and I just snipped a US power cord off something and wired it into the 4-gang socket.

I use two of these:

Not the cheapest, but I went through loads of cheap universal ones and they all broke. These have been fine for a long time now with no problems so I can highly recommend them. The euro plug on them also extends a long way, so no issues with the commonly found euro sockets that are recessed slightly.

For my blackberries & playbook I ditched the bigger UK supplied travel charger and bought a couple of these US supplied ones, which the blackberry USB/data cables plug into (US plug, so then plugs nicely into the above adapter):

My netbook power supply has interchangeable power adapters for different countries, so I ditched them all except the US one as takes up less room than the EU and UK one. If in the US, it plugs straight into wall- anywhere else, plugged into the second tumi adapter linked to above.

UK plugs take up a huge amount of room compared to US and are very awkward shaped so moving to all US plugs for me made a huge difference.

I made one of these a while ago;

With a US- and European-to-IEC320 lead (easily obtained, and often come free with printers and other computer equipment) you can use it in most parts of the world. And it's reliable and not wobbly in the socket. And you get to run four things off it ...

The 99p ones from eBay - then if I lose them I'm not bothered when I have to buy another - they work fine tbh

One of these for USB stuff (which is most things now):

Then a couple of the "anything to anything" cylinder type ones for laptop, camera chargers, etc.
