Surprised no one has mentioned Ellen MacArthur. I'd say the following is the short list:-

Overall: Flintoff, Vaughan, Gerrard, Lampard, MacArthur, Radcliffe, Montgomerie
Team: England cricket, Chelsea FC, Liverpool FC, Wales rugby
Young: Andy Murray
Overseas: Warne, Lara, Rossi, Dan Carter, Federer
Coach: Fletcher, Mourinho, Benitez
