Basically it's chicken, penne pasta, cream, mushrooms and tomatoes.

So how would you go about making it but with lovely home ingredients? it's the "creamy tomato sauce" bit I haven't done before, I assume I can just cook the pasta/chicken/mushrooms separately and then heat it all for about twenty mins in the oven in a dish?

Its like ready steady cook (without than annoying ahole Harriot)

Cook Penne al dente.
Chop chicken into pieces, season and fry in olive oil until coloured.
Add tinned tomatoes (use 2 tins for any more than 3 people) and mushrooms
Simmer for about 5 mins and check seasoning (longer if the toms have a lot of liquid in them)
Add creme fraiche
Stir mix into drained pasta
Stick in an oven dish and cover with Parmesan and (optional) seasoned breadcrumbs
Hot oven for about 10 minutes or so until top starts to brown.
Spoon into bowls, top with more parmesan and fresh basil.
Consume with Valpolicella.
