So you appeared before yourself?!

It is asking you to get a witness from a list of acceptable people (e.g. no doubt solicitor, doctor etc.) to complete section 5 confirming that you are who you are as per your driving licence.

Ah right, thanks for clearing that, I'll get it taken to my solicitor in that case.
