Cluncky diff...pain in the arse!

My diff or half-shafts are worn. After a fast run, setting off from junctions makes the rear of the car do a regular clunking sort of noise, which then disappears.

Anyone know what the half shafts and diff are off (Its got the Rover gearbox). Where I can get it reconditioned and how much etc?
