I can only tell the Officer facts -

"Hello Officer. The car in front had stopped. They say there was a vehicle swerved into their lane. I didn't see the other car"

Clearly that Officer didn't follow that up, and appears to have either been told a different story by V2 or, accepted the fact that V3 did swerve towards V2, but not recorded it.

Did he even note it?, The page layout and persistent presence of blank spaces as you turn the pages isn't due to a printing error.

I would make sure your concerns are duly noted, put in writing and able to be confirmed later should an unfortunate incident occur on the roads.

I don't know that it was noted.
The little information I received from what the Officer did report doesn't mention V3.

Unfortunately with the roadside CRaSH report it's very easy to "skip" steps. For example, I've only ever taken 1 roadside statement, skipping those sections with 'N/A'
