air expands in the sinuses on ascent and it should normally be able to bleed off through nasal passages etc. if it can't for any reason, the pressure exerted throughout the cavities is unequalized and is responsible for pain and sometimes bleeding.
it's the reverse of the "squeeze" divers can get if they descend too quickly.
aetiology is varied - polyps, upper resp tract infections, scarring, thickened mucosa etc. should get it looked at if it occurs this often as there are different "grades" and treatment could include some minor surgical interventions
